Description

Social workers play a unique role in the interdisciplinary care team. In best practice case management models, social workers work with psychosocially complex patients and family caregivers to provide psychosocial support, interventions and assistance with discharge planning.

This program will review how case management process can be used for social workers to organize their daily schedule. Our experts will review each step in the process and provide recommendations for social workers to engage with the interdisciplinary care team throughout the process.

In addition, this program will discuss the CoPs on discharge planning, home care referral, and working with patients and family care givers in a compliant environment. A detailed review of the role of a family caregiver will be reviewed in the program.

She has been active in the research and development of Case Management for over 20 years.

Beverly Cunningham -
Beverly Cunningham, MS, RN is a founding partner of Case Management Concepts, LLC. She has a 25-year deep working knowledge of case management with specific expertise in denials management, patient flow and the role of the Case Manager and Social Worker in the Case Management process. She has served as a Commissioner on the Commission for Case Management Certification and is a fellow with the Advisory Board. Bev is also the former Vice President of Resource Management at Medical City Dallas Hospital.
