Description

Over the past several decades, professional case management associations have identified the importance of effective transitions throughout the patient’s continuum. Effective transitions are wholly dependent on successful communication. Part of successful communication is an effective hand-off between transitioning providers of care.

It is equally important that case management and nursing focus on effective hand-offs. Recently, CMSA and the ACM have more definitively described the case manager’s role in effective transition management. During this webinar, we will review CMSA’s recommendations and the role of both the RN and social work case manager’s communication and hand-off as they effectively and efficiently transition hospital patients through the hospital and into the post-acute care continuum.

Objectives

Beverly Cunningham, MS, RN is a founding partner of Case Management Concepts, LLC. She has a 25‐year deep working knowledge of case management with specific expertise in denials management, patient flow and the role of the Case Manager and Social Worker in the Case
Management process. She has served as a Commissioner on the Commission for Case Management Certification and is a fellow with the Advisory Board.
