Description

Recognising the deteriorating patient is an essential nursing skill, and structured frameworks should be in place to assist effective patient assessment. The aim of this learning module is to encourage nurses to consider how to promote the 6Cs of nursing within such assessment. The module provides an overview of the Patient Assessment and Clinical-reasoning Tool designed to facilitate the development of clinical reasoning skills and effective communication with other healthcare professionals, thereby enhancing patient-centred care.

Objectives

On completion of this course you will be able to:
Understand and explain systematic assessment frameworks that can be used to enhance clinical decision making and to deliver patient-centred care.
Examine person-centred care underpinned by the 6Cs of nursing: care, compassion, courage, competence, communication and commitment.
Undertake a structured and systematic patient assessment.
Explore the effect of decision making on the provision of high quality individualised patient care.
Consider information gathering, history taking and documentation as essential components of the assessment process.
